1. Fax a medical prescription from your child’s doctor clearly stating your child’s name to 408-599-3020
2. You can also email us a copy of your child’s prescription here with your name and contact information.
3. Once you have faxed or emailed your prescription, contact a specialist at (800) 230-6775 to place your order.
4. If we need any additional information a specialist will call or email you.
